What started as a modest fundraising effort by three Lemont High School students – juniors Hope Conway, Olivia Matthews and Jenna Molaro – ultimately resulted in hundreds of Lemont students actively engaged in a humanitarian effort and tens of thousands of meals being shipped to a country across the world devastated by an earthquake. Athletics Forms

In order to participate in a sport at Lemont High School, all students are required to submit several documents. All of the documents - which are available to download on the right side of the page - must be turned in to the school's Athletic Office prior to the first day of practice for the student's sport. Failure to submit all documentation will result in the inability to participate.

The documents that must be on file in the Athletics Office before a student may participate in a sport include:

Athletic/Extra-Curricular Consent Form
This form must be signed both by the student and a parent/guardian. Students and their parents/guardians also are encouraged to sign the non-athletic extra-curricular section of the consent form if they plan on participating in any such activities.

IHSA Sports Medicine Acknowledgement & Consent Form
This form must be signed both by the student and a parent/guardian.

The Illinois High School Association (IHSA) employs a drug testing program in conjunction with all postseason competition. In accordance with this practice, Lemont High School consents to have any of its student-athletes tested for Performance Enhancing Substances (PES) during IHSA playoff competition. In order to participate during regular season or postseason play, all athletes (regardless of sport level) and their parents/guardians must consent to participate in the IHSA drug testing program, in case the athletes are selected for random testing during postseason play. Failing to consent to this testing prohibits a student from regular season or postseason competition.

The IHSA also requires that schools provide information to parents/guardians concerning concussion awareness.

Parents should read the first three pages of the IHSA Sports Medicine Acknowledgement document. The final page is the consent form, and it should be signed both the student and a parent/guardian.

IHSA Pre-Participation Medical Exam Form (Sports Physical)
All student-athletes must have a current IHSA Pre-Participation Medical Exam Form (i.e., sports physical) on file in the Athletics Office before participating. This form must be signed by the student, a parent/guardian, and the physician administering the exam. A student’s sports physical is valid for up to 13 months. In order to be eligible for the complete school year, the form should be dated after May 1.

Please note: All freshmen are required to have a State of Illinois Child Health Examination Form on file with the school prior to the first day of school. This form satisfies the requirement for the sports physical; freshmen should not submit the IHSA form to the school. The Child Heath Examination Form also  is valid for up to 13 months, and must be dated after May 1 of the student's 8th grade year in order for it to be valid for his/her entire freshman year.
