Health Examination and Immunization Requirements
In August 1996, the Illinois School Code was amended to allow school districts the option to require that students be in compliance with health examination and immunization requirements at an earlier date than October 15th, which is what is required by the State of Illinois. In effect, schools are allowed to exclude students who are not in compliance with health examination and immunization requirements. In the interest of the health and safety of our students, as well as the smooth, uninterrupted operation of classroom instruction for all students, Lemont High School has adopted such an earlier exclusion policy.
Parents and guardians should be advised that all required health and immunization records must be on file in the school office by the first day of school each year. Students whose records are not complete will not be allowed to attend school until all required health examination and immunization forms have been submitted to the school. The form for recording the required immunizations and examinations is available in the registration materials distributed by the school in the summer.
Lemont High School urges parents and guardians to schedule the appropriate medical appointments for their children early enough so that all immunizations and examinations are completed and documentation is on file in the school office by the first day of school.
