Lemont High School no longer is required to automatically mail Form 1095-C to individuals.

Current employees may access their 1095-C forms through Skyward Employee Access.

Individuals - including former employees - who are seeking a paper form may request the form by contacting Human Resources Manager Margaret Jazdzewski via:
